Activate Bookings
In order to book your child into the Activate After School Club or the Holiday Scheme you will need to do the following:
1. If your child is attending for the 1st time then please complete a registration form. Registration Form
- One form is needed per child.
- If your child attends regularly we will need to have a form completed once a year.
- If your contact details or personal information changes then a new form is required.
2. Be clear on the registration form which days of the week you want your child to attend.
3. Send your form in and wait for confirmation that a place is available. Please do not assume that your booking is secured until you have received written notification from LOPC.
- Payment can be made by debit or credit card by calling 0116 268 1426. We no longer accept personal cheques as a form of payment.
- If you wish to pay by Child Care vouchers then please contact us to confirm arrangements activate@lopc.co.uk

Cancelling Your After School Club Membership
You must given 4 weeks written notice to cancel your membership. Unfortunately, you will be charged for your child’s place during this period of time.
